The female lead, played by Park Eun-bin, challenges the audience's psychology with her portrayal of a mad doctor. The storyline shifts from revenge to murder mystery, sparking discussions among viewers. The classical music background during the scenes where she handles corpses creates a unique blend of elegance and terror that is unforgettable. Yoon Chan-young’s character, "Yeong-joo," unconditionally supports her after she saves his life, forming a strong reliance relationship. Park Byung-eun's character, anesthesiologist "Hyun-ho," serves as the last line of defense for the female lead, presenting great complexity with his moral dilemmas. Park Eun-bin's portrayal of the crazy doctor "世玉" showcases her manic acting skills for the first time, leaving viewers with goosebumps. The plot begins with revenge-driven medical practices and gradually evolves into a murder mystery, leading many viewers to express that "it's hard to sleep after watching." The character of 世玉, played by Park Eun-bin, exudes a terrifying charm, completely overturning traditional perceptions of a female lead. Most surprisingly, she enjoys playing classical music while handling corpses, creating a unique atmosphere that blends elegance and horror, which is also one of the strong contrasts in the drama.

In a significant scene, 世玉 hums classical music while calmly burying a body at a bloody crime scene, resulting in visuals that are both shocking and filled with a strange aesthetic, making it impossible for viewers to look away. Besides the main characters played by Park Eun-bin and Seo Kyung-soo, there are many talented actors joining the cast. Yoon Chan-young's character "榮柱" has sparked much discussion, as he was once saved by 世玉 and holds gratitude towards her, seeing her as the only ray of light in his life. Although he continuously complains on the surface, he actually unconditionally supports all of 世玉's decisions.

After 世玉 commits murder, 榮柱 goes so far as to strip off his clothes to help her clean the crime scene. Although he repeatedly exclaims, "You’re really crazy," he harbors the determination of "only I can protect you." In an interview, Yoon Chan-young stated that his character is like a "little bird chasing its mother," relying on 世玉, and can treat a corpse as casually as dealing with "cockroaches at home." When 世玉 casually asks, "Do you like me?" 榮柱 answers with a sense of grievance, "Yeah, don't you like me?" This innocent reaction leaves viewers anticipating the future developments between the two.

In contrast to 榮柱's emotionally rich and impulsive performance, the anesthesiologist "賢浩," played by Park Byung-eun, serves as 世玉's last rational defense. He not only perfectly assists 世玉 in every illegal surgery but also timely intervenes or cleans up when she is about to lose control. Park Byung-eun's character is a being that is "both kind and contradictory"; he admires 世玉's skills in saving lives while simultaneously assisting her in executing various illegal procedures, wanting to protect her from harm yet tacitly approving of her murderous acts, making 賢浩 one of the most complex characters in the drama.
